For several years we have worked with our partners in local authorities and the third sector to agree an improved way to ensure a smooth transition pathway for people with Additional Learning Needs.
Please check out the Transitional planning proposals and take a few minutes to share your opinions - these are being signed off soon so please let us know any improvements you feel need to be made

This survey is being conducted by the City of Cardiff Council, Vale of Glamorgan Council, and Cardiff and Vale University Health Board (the local NHS).
We want to improve the way we work together to support transition planning to further education and/or adulthood for children and young people with Additional Learning Needs (ALN) living in Cardiff and the Vale of Glamorgan.
We have created a Multi-Agency ALN Transition Protocol which explains how we will do this to make sure:
- The views, feelings and wishes of children, young people, their families
and carers are fully heard when planning for the future
- Children, young people, their families and carers have information to
help them to make choices
- There is a joined-up approach to early planning and preparation for
transition to adulthood and the right people are involved.
We would like your views on this protocol and also want to:
- Know about your experiences of transition;
- Understand any gaps in existing provision, information, advice and
support.
